Warrior Ceremony Form

When an apprentice is of proper age {12 moons} and has completed every step of their training, the member who plays their mentor can fill out this form and either PM Tαℓвσт or that particular Clan's Supervisor. **Optional: the apprentice, being aware that their mentor is doing this, can offer one name they would fancy as their warrior name and the reason behind it. You will see the space for this below in the form.

Once the staff member has been PM'd, following the naming guidelines, they will generate a list of 10 names (more or less): 5 having to do with their physical strengths, 5 others having to do with their appearance. If the apprentice's suggested name is reasonable, it will take up the space in either an attribute-based-name or an appearance-based-name (whichever category it falls under). This list will be relayed to the Clan leader, who will then announce the apprentice's new name during the ceremony.
